Job Description

To assist the Transition Planning Manager to establish good work processes and systems to manage and execute effective Transition planning functions in supporting students in the attainment of post school Living, Learning and Working outcomes according to stated Key Performance Indicators (KPI).

Operational Excellence

  • Co-ordinate and manage the Transition Planning processes and ensure implementation timeline is adhered
  • Collate and analyse parents and students Transition Planning survey results
  • Manage and maintain updated database of student’s Individual Transition Plan (ITP)
  • Monitor and ensure information in ITP is updated annually
  • Co-ordinate sharing and briefing sessions on ITP for parents
  • Co-ordinate ITP briefing and training sessions for staff
  • Co-ordinate and manage Transition Planning Meetings to allow relevant parties (Parents, teachers, external agencies) to come together to plan the ITP and discuss post school options and pathways with the student
  • Monitor implementation of activities and supports for the students and update in students’ ITP
  • Maintain collaterals with updated information on pathways and referral processes for post-school options


Fruitful Collaboration

  • Co-ordinate and manage the Transition Planning processes and ensure implementation timeline is adhered
  • Collate and analyse parents and students Transition Planning survey results
  • Manage and maintain updated database of student’s Individual Transition Plan (ITP)
  • Monitor and ensure information in ITP is updated annually
  • Co-ordinate sharing and briefing sessions on ITP for parents
  • Co-ordinate ITP briefing and training sessions for staff
  • Co-ordinate and manage Transition Planning Meetings to allow relevant parties (Parents, teachers, external agencies) to come together to plan the ITP and discuss post school options and pathways with the student
  • Monitor implementation of activities and supports for the students and update in students’ ITP
  • Maintain collaterals with updated information on pathways and referral processes for post-school options
  • Liaise with industry partners to facilitate learning journeys, work attachment and internship
  • Liaise with Institute of Higher Learning to facilitate learning journeys, learning days, workshops and seminars etc
  • Liaise with external agencies (E2C, MSF, Sg Enable, other post school service providers) to facilitate support needed by students

Any other duties as assigned by the Principal when necessary


Requirements:

  • A good Bachelor’s Degree in any field
  • At least 1-3 years relevant working experience
  • Good track record in administrative functions
  • Good command of verbal and written English
  • Ability to work in a fast paced, dynamic and challenging work environment
  • Competent in Microsoft Office (especially Word, Powerpoint and Excel)
  • A genuine desire to serve the special needs community
