Job Scope: Temperature taking and collect declaration form.

1.      Have you travelled to, and/or returned from China, Japan, Switzerland, UK, Iran, Italy, France, Germany, Korea, Spain and ASEAN countries (which include Brunei, Cambodia, Indonesia, Laos, Malaysia, Myanmar, Philippines, Thailand and Vietnam)? (YES/NO)

2.      If yes, please state the start date and end date of the travel

3.      If yes, please state date and countries transited in.

4.      To your knowledge, do you have fever (≥ 37.5°C) or other symptoms such as cough, flu, sore throat and breathlessness? (YES/NO)

5.      Have you had close contact with a confirmed case of CORONAVIRUS DISEASE 2019 (COVID-19) within the past 14 days? (YES/NO)
