Job Description

Responsibilities:

            Patient Care

  • Provide counselling to patients including education on chronic disease management (CDM) and health promotion
  • Teach specific tests for monitoring of patient’s progress (e.g. use of medical devices, blood pressure, blood glucose, inhalers, glucometer and send results to physician to review)
  • Track patient’s progress in CDM and lifestyle changes (may involve telephone consultations and telehealth monitoring)
  • Identify patients with “at-risk” clinical indicators for closer management and supervision by working with the individual to develop, monitor and revise tailored health plans according to established standards and protocols
  • Track and analyse patient’s results, as well as plan and conduct telephone consultations with patients to monitor and assess needs
  • Escalate cases to the doctors following PCN (Primary Care Network)’s escalation protocol.
  • Conduct DFS, DRP and nurse counselling


Community

  • Work closely with the Primary Care Coordinators (PCCs) in care coordination and care transition. This will involve liaising with the public institutions, clinic doctors and clinic assistants, as well as community health and social providers
  • Provide input on clinical results of the various clinics in the PCN
  • Review/revise nursing guidelines, standards and procedures related to chronic disease management
  • Comply with universal precautions and adhere to all infection control practices in the delivery of nursing care
  • Maintain confidentiality of patient medical records at all times
  • Any other ad-hoc duties by the Project Manager

 

Job Requirements:

  • Registered nurse with SNB and minimum 3 years working experience in chronic disease management
  • RNs with >5 years continuous experience in other specialties may be considered
  • Candidates with advanced diplomas in Chronic Disease Management will be at an advantage
  • Able to travel around from clinic to clinic
  • Ability to speak Mandarin and Dialect is preferred as clinics will be handling elderly, non-English speaking patients
  • Nurses dealing with dialysis as chronic disease will also be considered
