Day Care / Homecare Staff Nurse *Office Hours 5 days*

Duties:

• Perform pre-assessment of new clients
• Ensure the clients’ enrolment complies with the Company’s SOP
• Administer the documentation to be submitted to relevant government body
• Assess and coordinate with health care team to assess, plan, implement and evaluate client’s conditions
• Monitor, assess and evaluate clients’ condition to implement a care plan for the client
• Educating clients and families
• Provide health promotion, counselling and education
• Perform nursing care such as medication administration and wound dressing 
• Oversee and manage the performance of the nursing team to ensure desired level of work competency is achieved
• Coordinating, implementing and monitoring clinical and service quality performance indicators for continuous improvement
• Responsible for assessing the needs and delivery of quality geriatric / dementia nursing care to clients

Requirements: 

• Min Diploma / Degree in Nursing 
• Registered with Singapore Nursing Board
• Able to start immediately or on short notice period.
