Contract Social Work Associate, up to $2.9K

Jobscope:

• Manage ESRD patients in dialysis centre or home dialysis through client engagement, conducting interviews, data collection, case assessment, case conceptualisation, case planning and intervention plans for best patient outcomes
• Identify underlying issues and problems to design a care plan to enhance and promote psychosocial functioning of patients
• Conduct visits to dialysis centre, home or hospitals to provide supportive counselling
• Collaborate with healthcare professionals. internally and externally in total patient management
• Other duties as assigned

Requirements:

• Min Diploma in Social Work or other related duties
• Min 2 years of experience
• Able to start work immediately
