Contract - Perm Therapy Programme Assistant

Job Scope:

• Have to lift up / support elderly to the toilet as they might be physically disabled.
• Perform tasks and daily activities with the elderly according to the care plans developed by clinical team
• Maintain up-to-date documentation of client’s conditions and interventions and support development and review of care plans
• Ensure client’s integration and recommend measure to mitigate risk to the clients
• Conduct activities for clients to promote independence, health, wellness and quality of life
• Provide guidance to clients on personal, social or work skills

Requirements

• Min GCE N/O levels or Nitec in Community Care & Social Services and any related fields
• Prior experience in healthcare would be an added advantage
• On-job-Training provided
• Candidates have to be comfortable in handling dementia patients
